Who We Are
SERVE is prescreening individuals for the Community Pharmacy in the Moss Clinic. The Community Pharmacy Prescription Program helps provide medications that are needed on a continual basis for those who can't afford them.
SERVE will pay the initial program fee of $50.00 for you. You must bring the following items with you to your prescreening appointment (they are mandatory for admittance into the Community Pharmacy Prescription Program):
Proof of any household income
Valid driver's license or ID
Current 1040 Federal income tax return
W2 forms
3 pay stubs from your current employer
Food stamps notice of action
Social Security statement
Letter of support
Original prescriptions
Your Physician must fax the following documents to begin the process:
Your name; demographic information; prescription information including dosage and quantity; and list of known allergies.
