Special Care Organizational Record (SCOR)

The Special Care Organizational Record (SCOR) is an organizing tool for families who have an adult family member with special health care needs. This includes spouses and adult children with special health care needs, as well as other adult dependent family members. It is designed to help you keep track of all of the relevant information regarding your family member’s health and care.

The SCOR will help you organize all of this information in one place and make it easier for you to quickly find what you need. It will also make it easier for you to share key information with those who are part of your family member’s care team.
