Event Description

What if a sudden illness or injury left you unable to speak for yourself? Who would you want to speak for you? What would you want them to know about your values and wishes? Advance Care Planning (ACP) is a conversation process about the medical care you would want if you could not speak for yourself, and needed others to make decisions for you. Advance care planning helps you think about health care choices you may have to make in the future; talk about your choices with your doctor and your loved ones; and make a written plan for the future (advance directive). This session is scheduled to take place in person and be broadcast online. It may be moved to a completely virtual format, pending COVID-19.
Audience: Those who are interested in learning about the importance and process of person-centered ACP -- Those who want to learn how to start an ACP conversation with a loved one in order to honor their wishes -- Those who want to empower others with information about ACP.
